This form should only be used by currently admitted and active graduate students seeking to change their current graduate degree program within your current college.
If you are changing graduate programs across colleges, are a certificate student or a post-bac non-degree seeking student, you must submit a new application through Graduate Admissions.

By making this request for a change of graduate degree program, I authorize the faculty in the new graduate degree program to review my complete file in the process of making a decision regarding my admission to the new graduate degree program. I have reviewed the admission requirements for the new graduate degree program and have attached to this request any additional information required for the new graduate degree program and other additional information that I would like the members of the admissions committee to consider. I understand that meeting the minimum requirements for admission is no guarantee that I will be admitted to the new graduate degree program. If original documents such as transcripts and test scores are needed, it is my responsibility to make the necessary arrangements to have them sent directly to the academic unit of the requested new graduate degree program.

If this request is approved, I am confirming abandonment of the current degree program and catalog year to pursue the new degree program and the new catalog year. I understand that I may not be allowed to revert to the previous graduate degree program after this request is processed. This also applies if my request includes a change of degree level (Masters to Doctorate or Doctorate to Masters).

By submitting this form, it will be routed for approval by first my current program coordinator and second my requested program coordinator.
